fix(wiki): lint 修复7处缺陷

- 创建 wiki/meta/ 目录，移入 index.md 和 log.md
- 补全 index.md、log.md 缺失的 created/updated 字段
- 修复所有 wikilink 路径，去除 .md 后缀（Obsidian 规范）
- 在 CLAUDE.md 添加 meta 到 type 枚举
- 建立 focus-core、top5爆款source、三公里source 的交叉引用，消除孤立页面
- topic 页第一节新增对 top5 source 的显式来源标注
