feat(server): add Prisma schema with all 8 data models
- User, CardType, Membership, WeekTemplate, TimeSlot, Booking, Order, StudioConfig
- PrismaService and global PrismaModule
- snake_case column mapping with camelCase TypeScript fields
- Proper indexes and unique constraints
